Ocarina of Time

Equipment

Kokiri Sword: hidden away in a chest inside the Hole of "Z" in the Forest Training Center.
Master Sword: in the Temple of Time.
Biggoron Sword: Biggoron gives it to you after you complete the Trading Sequence
Deku Shield: In the Kokiri Shop for 40 Rupees
Hylian Shield: In the Bazaar at Hyrule Castle Town, and can be found in the Kakariko Village Graveyard underneath a grave.
Kokiri Tunic: Starting gear
Goron Tunic: Can be bought in the Goron Shop for 200 Rupees, or gotten as a gift from Link (Goron)
Zora Tunic: Can be bought in the Zora Shop for 200 Rupees, or gotten as a gift from King Zora
Kokiri Boots: Starting gear
Iron Boots: found in the Ice Cavern near Zora's Fountain

Basic Items

Deku Sticks and Deku Nuts: Pretty much anywhere, in some shops, and Link can get them by destroying Deku Baba
Bombs: Most shops, and Link can get them by destroying some enemies
Bombchus: Some shops, and Link can get them by destroying some enemies
Magic Beans: purchased from a man sitting near the Zora's River
Fairy Ocarina: Given to Link by Saria when you leave Kokiri Village
Ocarina of Time: Given to Link by Princess Zelda during her escape from Ganondorf
Din's Fire: A Gift to Link from the Great Fairy near Hyrule Castle
Farore's Wind: A Gift to Link from the Great Fairy in Zora's Fountain
Nayru's Love: A Gift to Link from the Great Fairy near the Desert Colossus
Goron's Bracelet: A Gift to Link from the Gorons
Fire Arrows: Found in Lake Hylia by shooting an arrow to the sun during dawn.
Silver Scale: A prize Link wins from in Zora's Domain when he beats the Diving Game.
Golden Scale: Awarded to Link when he catches at least a 14 pound fish in the Fishing Hole

Majora's Mask

Main Items

Masks: See here for a complete list of mask locations. With the exception of the Boss Remains, Mask of Truth, Giant's Mask and Fierce Deity's Mask, every mask in the game can be found in the overworld, particularly in Clock Town.
Ocarina of Time: Seized from Skull Kid during the last minutes of the Final Day.
Bombs: Obtainable everywhere after purchasing a Bomb Bag in either the Bomb Shop or in Curiosity Shop, both located in Clock Town.
Deku Nuts and Deku Sticks: As in the case of Ocarina of Time, these items can be found in several places, including defeat of Deku Babas and in shops. On the other hand, the frequency of these items is somewhat lower.
Pictograph Box: Given by Koume as a present in the Swamp Tourist Center.
Magic Bean: Both a Business Scrub near the Swamp Tourist Center and the Bean Seller inside the grotto in Deku Palace have beans for sale.
Lens of Truth: In Lone Peak Shrine.
Powder Keg: Obtainable for the first time in Goron Village by Medigoron.
Bombchu: Located in several chests across Termina, and for sale in Bomb Shop as well.
Hookshot: Pirates' Fortress.
Great Fairy's Sword: Given as a reward for rescuing all Stray Fairies from Stone Tower Temple.
Bottles Unlike the rest of the games in the series, which feature up to four, Majora's Mask has up to six to be collected. See here for info on how to find them.

Equipment

Bombers' Notebook: There are two ways to obtain it: Link can either play the Hide and Seek game with the Bombers in Hylian form, or simply exit from their hideout in Hylian form.
Kokiri Sword: Starting gear, but it can only be used after Link recovers his Hylian form.
Hylian Shield: Starting gear, but it can only be used after Link recovers his Hylian form.
Razor Sword and Gilded Sword: For 100 Rupees, Zubora and Gabora can forge the Kokiri Sword to turn it into the Razor Sword, which will revert into its former shape after 100 slashes or Link plays the Song of Time. With Gold Dust, the Razor Sword can be upgraded to the permenent Gilded Sword.
Mirror Shield: At the end of the Gibdo well.

Outset Island Items

Hero's Clothes and Hero's New Clothes: Depending on whether the First or Second Quest is started, Link will receive either clothes from his grandmother in Outset Island.
Telescope: Given by Aryll in Outset Island.
Sword: Given by Orca in Outset Island, later retrieved again in Forsaken Fortress.
Shield: Given by Link's grandmother in Outset Island.

Windfall Island Items

Sail: For sale by Zunari, for only 80 Rupees.
Tingle Tuner: Given by Tingle after freeing him.
Picto Box: Behind Tingle's jail, past a tunnel maze.
Bombs: Inside Tetra's Ship, after Link completes Niko's last challenge.
Magic Armor: Given by Zunari after the Island Trading Sequence.
Hero's Charm: Given by Mrs. Marie after giving her 40 or more Joy Pendants.

Other Items

Bait Bag: For sale in Beedle's shop ship for only 20 Rupees.
Spoils Bag: Inside Tetra's Ship, after Link completes Niko's first challenge.
Wind Waker: Given by the King of Red Lions in Dragon Roost Island.

Deku Leaf: Near the top of the Great Deku Tree in Forest Haven.

Wooden Sword: Given by Rusl as a present. Later given to Talo.
Ordon Sword: Inside Rusl's house, obtainable by Wolf Link the night of the day when the children from Ordon Village are abducted.
Master Sword: Sacred Grove.
Ordon Shield and Wooden Shield: Inside Rusl's house, obtainable by Wolf Link the night of the day when the children from Ordon Village are abducted. If fire burns it, it's lost forever, and Link will then have to purchase a Wooden Shield in a shop.
Hylian Shield: For sale in Malo Mart for only 100 Rupees.
Hero's Clothes: Given by Faron after retrieving the Tears of Light in Faron Province.
Zora Armor: Inside the tomb of King Zora in Kakariko Village Graveyard.
Magic Armor: For sale in Chudley's Fine Goods and Fancy Trinkets Emporium for 100000 Rupees, later for 598 Rupees when it's rebranded as a branch of Malo Mart.

Fishing#Fishing Rods: Given in Molida Island as a reward for completing the Mermaid Sidequest.
Phantom Sword: Given in Zauz's Island after collection of all Pure Metals.
Swordsman's Scroll: Given in Molida Island after completion of the Phantom Hourglass Trading Sequence.

Stamp Station Rewards

Shield of Antiquity: Reward for collecting 10 stamps.
Engineer's Clothes: Recovered for collecting 15 stamps.
Swordsman's Scroll 2: Reward for collecting 20 stamps.

Recruit Uniform: Hyrule Castle, inside the room of Princess Zelda.
Recruit's Sword: Hyrule Castle, given by Guard Captain Russell after completion of the Sword Training game.
Spirit Flute: Hyrule Castle, inside the room of Princess Zelda.
Bombs: Obtainment depends of where the first Bomb Bag is collected (either Beedle's Air Shop, Hyrule Castle or Whittleton).
